Policies

Eligibility
Regardless of home campus, USF students who are currently registered for classes are eligible to receive services at Student Health & 911±¬ÁÏÍøllness Center (SHWC). You will need to present your current, valid USF student ID card when you come into the clinic. If you are taking a semester off (e.g. enrolled for Fall 2024 but not taking classes Summer 2024), you can access services at SHWC during the off-semester if you pay a Health Fee. USF graduates who can show proof they will be continuing in a USF program within 2 semesters following graduation may also voluntarily pay a health fee to access services between enrollment.

Fees for missed appointments (No Shows), late cancellations or late arrivals.
Effective January 1, 2025, the Student Health & 911±¬ÁÏÍøllness Center (SHWC) will assess a fee for missed appointments (No Shows), late cancellations or late arrivals. SHWC provides a full range of primary care to enrolled students on an appointment basis. Since there are costs associated with not utilizing a scheduled appointment due to a patient’s lack of timely notification/arrival, the No Show/Late Arrival Fee will be assessed. Fees posted to Student Self Service may generate an administrative hold.

No Show Fee
If you have an appointment that you cannot keep, please contact the Student Health & 911±¬ÁÏÍøllness Center (SHWC) no later than 12 noon (pm) the business day before your appointment time to cancel or reschedule via the patient portal (MyChart) or by calling during business hours. If you fail to cancel and do not show up for your scheduled time, you will be assessed a No Show fee on Student Self-Service (formerly Student OASIS).

Note: For dermatology appointments, the patient needs to cancel by 12 noon two business days prior to their appointment time.

Late / Tardy
Late Arrival is defined as when a patient with a scheduled appointment arrives 7 minutes after their scheduled appointment time. 

When you are late, it causes other scheduled appointments to be delayed, which isn't fair to other patients. Therefore, if you arrive more than seven (7) minutes late for your scheduled appointment, you will be rescheduled and you will be assessed a Late Arrival fee on Student Self-Service (formerly Student OASIS) .

911±¬ÁÏÍø recommend that you arrive 10 to 15 minutes ahead of your scheduled appointment to ensure you have plenty of time to find parking, check in, complete any necessary paperwork and relax for a moment before seeing your provider.

Medical Excuses
911±¬ÁÏÍø may experience medical conditions, psychological conditions, illnesses or injuries that require them to be away from the classroom or workplace, or that affect their ability to perform optimally in class or on exams.

Per USF policy, all students must comply with the provisions of the . Attendance is the responsibility of the student and instructors have the discretion to approve or disapprove any absence. Although SHWC may verify that you received care and offer a statement in support of consideration of an excused absence, only the instructor may excuse an absence. Visitors
Unless you are a patient or a friend or family member escorting a patient, you will need to sign in to our Visitor Log at the front desk before entering any clinical areas.

Immunization Compliance
The 911±¬ÁÏÍø requires the following immunizations of all students:

MMR: All students born on or after January 1, 1957 must show proof of TWO vaccinations for Measles and Rubella

MENINGITIS: All students must show proof of vaccination or sign a declination.

HEPATITIS B: All students must show proof of vaccination or sign a declination.
